STUDENT ENROLLMENT REQUIREMENTS
1) Must be a minimum of 16 yrs old (Student applicants under 18 yrs of age must be accompanied by a parent or legal guardian at the time of enrollment)
2) MUST submit Two forms of ID: Active driver license OR Active State ID OR valid government issued ID, AND birth certificate OR valid U.S. passport OR valid permanent resident card (green card) OR valid permanent resident card (green card) OR valid USCIS work authorization card.
3) Original Social Security Card
4) Copy of High School Diploma or GED or pass an entrance exam in lieu of diploma
5) Complete Health Examination and Tuberculosis Screening (TB) with negative PPD results and/or Chest X-Ray (chest x-ray must have proof of positive PPD results with an official radiology report) Date of record of health examination/TB and/or chest X-ray/radiology report must be within 90 days of the program end date and must be completed and signed by a Physician, Physician Assistant or Nurse Practitioner .
6) Proof of Flu vaccination or an exemption letter from a licensed physician (date of record of Flu vaccination must be within 1 year of the training program end date.
7) Proof of COVID-19 Vaccination or declination letter or physician exemption letter
8) Complete a Live Scan/Criminal Record Screening (must be completed prior to the start of the training program)
9) American Heart Association (AHA) Basic Life Support (BLS) Certification (Online CPR/BLS and other BLS certifications not from AHA will not be accepted) *BLS Certification must be completed prior to the start of the program. (ask an ACI representative about BLS training)
ALL of the original documents for the enrollment requirements must be hand-delivered to a school official upon enrollment and by appointment only. Documents must be hand-delivered and submitted in person at the main school office located at 2445 W. Chapman Ave. Suite 210, Orange, CA 92868
(Emailed copies, photocopies, photos or mailed copies of documents will NOT be accepted)

1. Valid driver’s license OR valid state ID OR government issued ID
2. Original Social Security Card
3. Health Examination (physical exam) AND Tuberculosis Screening (TB test) with negative PPD results and/or Chest X-Ray. (Health Examination and TB test or chest x-ray must be within 90 days of the program start date and must be signed by a Physician, Physician Assistant or Nurse Practitioner.
4) Flu vaccination record (date of record must be within 90 days of the program start date)
5. A currently active and verifiable CNA certification card issued by The California Department of Public Health (CDPH). Copy of the nurse assistant training program certificate of completion. (for recent graduate students)
Refund Policy: Tuition and Fees are ALL non-refundable/non-transferable)
Tuition and Fees are due in full upon enrollment
Program Fee : $10 Per Unit, minimum of 4 hours
