CANCELLATION, WITHDRAWAL or DISMISSAL
If a student cancels or withdraws from the enrollment agreement for any reason prior to starting the program or through the course of the program will be subject to the school’s non-refundable fees policy. In the event a student cancels, withdraws, or is dismissed from the program due to absenteeism and/or violation of school policies and/or student misconduct, the tuition and registration fee paid to the school and third-party miscellaneous fees are non-refundable.
NON-REFUNDABLE FEES POLICY
The following fees and costs are non-refundable: registration fee and tuition and payments made to Third Parties such as; textbooks, uniforms, state exam fee and live scan fee.
ATTENDANCE POLICY
A student can only be absent no more than one (1) day for theory training (excused or unexcused) and can only be absent one (1) day for clinical training (excused or unexcused). A tardiness will count towards the one (1) day absence for theory training or one (1) day absence for clinical training. If a student arrives late, the student will not be allowed to enter class and will be recorded as absent and is subject to a full-day make-up day. If a student returns late from lunch break, the student will not be allowed to return to class, the student will be recorded absent and will count towards the one (1) day absence for theory or one (1) day absence for clinical training and will be subject to a full-day make up day (per the make-day policy) A full day make-up day will consist of 8.5 hours (includes lunch break) for the weekday program schedule or 8.5 hours (includes lunch break) for the weekend program schedule or 4 hours (no lunch break) for the evening program schedule. A student’s request to leave early before the class ends with an instructor’s approval will be recorded absent and will be subject to a full-day make make-up day (per the make-up day policy) A student’s request to leave class early before the class ends with an instructor’s approval will be recorded absent and will be subject to a full-day make-up day, and the absence does count towards the one (1) day absence for theory training, or one (1) day absence for clinical training. (per the make-up day policy) There are NO make-up days allowed for the home health aide program and continuing education class. Absences and/or tardiness from the home health aide program and continuing education class will result from the immediate dismissal from the home health aide training program and continuing education course.
GRADUATION REQUIREMENTS
TO MEET THE GRADUATION REQUIREMENTS, THE STUDENT MUST COMPLETE THE FOLLOWING:
- Complete required 60 hours of Theory Training and 100 hours of Clinical Training.
- A grade of 75% or better in all theory modules and exams.
- Passing clinical evaluations with satisfactory performances, demonstrating that the student has successfully completed all clinical objectives.
- Tuition and fees paid.
CLASSROOM OR ONLINE ASYNCHRONOUS THEORY SESSION AND IN-PERSON CLINICAL TRAINING
Theory classes are held at the Allegiance Career Institute main campus located at: 2445 W. Chapman Ave., Suite 210, Orange, California. Online Asynchronous Theory training will be online through the school’s Online Learning Management System. Clinical site training locations are held only by approved CDPH clinical sites for Allegiance Career Institute.
DRESS CODE POLICY
The dress code policy at Allegiance Career Institute is strictly enforced to maintain an image of professionalism. It is the expectation of Allegiance Career Institute that a professional atmosphere is always maintained. Therefore, students should exercise good judgment in their clothing apparel.
The following dress code is strictly enforced:
Allowed:
- School issued scrubs with school logo, uniform must always remain completely visible. Clean and ironed with no wrinkles.
- Plain white rubber athletic shoes, nursing shoes, shoes must be kept clean.
- Plain white socks, plain white undershirts (long sleeve or short sleeve)
- School issued ID badge must always be visibly worn. ID badge must be kept in a presentable good condition.
- Hair must be clean and neatly groomed off the collar.
- Nails must be clean, trimmed and moderate length.
- Body deodorizer or deodorant is essential.
- Arm sleeves (must be plain white)
- Long hair must be pulled back and tied in a bun with simple hair ties/barrettes.
- Well-groomed beards and moustaches no more than 1 ½ inch in length)
- Modest and light make-up
- Jewelries: ring with NO stones, small stud earrings (one on each ear), analog wristwatch.
NOT Allowed: (NO EXCEPTIONS)
- NO Hooded sweatshirts, jackets, sweaters, T-shirts
- NO clogs, sandals (crocs), slippers and high-top shoes
- NO colognes and perfumes
- NO COLORED NAIL POLISH OR CLEAR POLISH
- NO ARTIFICAL OR ACRYLIC NAILS
- NO decorative clips, ribbons, scarves, bandanas, hats, baseball caps
- NO heavy harsh make-up
- NO large loop or dangling earrings, bracelets and/or necklaces.
- NO smart wrist watches (eg; Apple watch, Samsung watch, etc.)
- NO Nose, tongue or lip piercings and other facial piercings. Must be removed.
- Tattoos: Any visual tattoos must be always covered while in campus or clinical training site.
STUDENT ACKNOWLEDGEMENT (DRESS CODE POLICY)
I acknowledge that I have fully read and understand the school dress code policy and will adhere to this school policy. I fully understand that if I violate the dress code policy I will not be allowed in the classroom or clinical training site and may lead to further disciplinary action including up to immediate dismissal from the program.
Student Acknowledgement:
By signing below, I acknowledge that I completely understand and I am aware of the school’s make-up day policy and fee. I do hereby acknowledge that I will adhere to this policy. I agree that Allegiance Career Institute has the right to implement any changes to the requirements of this policy in its sole discretion. Failure to meet the make-up day policy and requirements will result in disciplinary action including up to dismissal from the training program.
Photo/Video Release Form
For good and valuable consideration, the receipt of which hereby acknowledged, I, hereby grant Allegiance Career Institute permission to use my likeness in a photograph and/or video and in any and all of its publications, including but not limited to all of Allegiance Career Institute’s printed and digital publications, identification purposes such as identification badges and for online learning management system identification. I understand and agree that any photograph using my likeness will become property of Allegiance Career Institute and will not be returned.
I acknowledge that my participation with Allegiance Career Institute is mandatory, I will receive no financial compensation.
I hereby irrevocably authorize Allegiance Career Institute to edit, alter, copy, exhibit, publish or distribute this photo and/or video for purposes of publicizing Allegiance Career Institute’s programs or for any other related, lawful purpose. In addition, I waive the right to inspect and approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph and/or video.
I hereby hold harmless and release and forever discharge Allegiance Career Institute from all claims, demands, and causes of action which, I, my heirs, representatives, executors, administrators., or any other persons acting on my behalf of my estate have or may have by reason of this authorization.
VACCINATION POLICY
INTRODUCTION:
The purpose of this policy is to adhere to the California Department of Public Health guidelines and to minimize transmission of viruses in the community. This policy will provide protection to students and preventing community transmission. The following vaccination will be required for Allegiance Career Institute.
- COVID-19 Vaccination and Booster Shot
POLICY:
Allegiance Career Institute requires that students maintain up-to-date vaccinations to protect the health and safety of patients, personnel, visitors, and the community from infection.
Students must provide proof that they have received the complete COVID-19 vaccination with a booster shot. A student must maintain a copy of the vaccination records, which the student must provide upon the school’s request.
Students may be exempt from the vaccination requirements only upon providing a declination form, signed by individual stating either of the following: (1) the student is declining vaccination based on Religious Beliefs, or (2) the worker is excused from receiving any COVID-19 vaccine due to Qualifying Medical Reasons.
To be eligible for a Qualified Medical Reasons exemption the worker must also provide to their employer a written statement signed by a physician, nurse practitioner, or other licensed medical professional practicing under the license of a physician stating that the individual qualifies for the exemption (but the statement should not describe the underlying medical condition or disability) and indicating the probable duration of the worker’s inability to receive vaccine (or if the duration is unknown or permanent, so indicate).
To be eligible for an exemption for Religious Beliefs, a student must have a sincerely religious belief, practice, or observance that prevents the student from taking any of the FDA authorized or approved COVID-19 vaccines. Religious beliefs can include unique beliefs held by few or even one individual; however, mere personal preferences are not religious beliefs. Religion includes all aspects of religious observance and practice as well as belief; not just practices that are mandated or prohibited by a tenet of the individual’s faith. Religions includes not only traditional, organized religions such as Christianity, Judaism, Islam, Hinduism, Sikhism, and Buddhism, but also religious beliefs that are new, uncommon, not part of a formal church or sect, only subscribed to by a small number of people, or that seem illogical or unreasonable to others.
If a student is deemed to have met the requirements of an exemption the unvaccinated exempt student must meet the following requirements when entering a long-term care facility:
- Test for COVID-19 with either PCR (Polymerase Chain Reaction) or antigen test that either has Emergency Use Authorization by the U.S. Food and Drug Administration or be operating per the Laboratory Developed Test requirements by the U.S. Centers for Medicare and Medicaid Services. Testing must occur twice weekly for care and long-term care settings, and once weekly for such students in other health care settings.
- Wear a surgical mask or higher-level respirator approved by the National Institute of Occupational Safety and Health (NIOSH), such as an N95 filtering facepiece respirator, at all times while in the facility.

Failure to meet this policy may result to disciplinary action including up to dismissal from the program.
COVID-19 TESTING POLICY
As a condition of enrollment to Allegiance Career Institute’s training programs. Students must adhere in accordance to the school’s Infection Control and Safety measures policy to ensure the safety, well-being and health of students, school faculty and clinical training sites. The purpose of this policy is to conduct stringent measures to protect safety and health of the community and to prevent the spread of COVID-19. Allegiance Career Institute has the right to request for COVID-19 testing with proof of negative results if the student shows any signs and symptoms of the following:
- Fever of 100.4 or higher
- Cough
- Shortness of Breath
- Sore Throat
- Loss of smell/taste
- Diarrhea/nausea
Allegiance Career Institute will adhere to the federal and state guidelines to COVID-19 quarantine, isolation and testing policies.
When a student attends clinical training in a long-term care facility, a student must adhere to undergo COVID-19 testing in order to participate in the clinical training of the program and adhere to the clinical facility policies and COVID-19 testing policy of Allegiance Career Institute. Failure to adhere to said policies may lead to dismissal from the training program.

MANDATORY INFLUENZA (FLU) VACCINATION POLICY
As a condition of enrollment to Allegiance Career Institute’s training programs. It is thereby mandated that a student must receive the annual influenza vaccination requirement or must possess an approved exemption from a licensed physician.

By clicking the “I Accept” checkbox, I acknowledge that I have read the required influenza vaccination policy and I fully understand that I am required to meet this requirement as a condition to enrollment to Allegiance Career Institute’s training programs. I acknowledge that if I do not meet this requirement, I will be subject to dismissal from the training program.
